Beginning Tutorial - Multiple Page Form

To create a multi-page form:
  1. Open your survey form.

  2. Go through your form and put a Page Break where you want your page breaks to be. A page break will automatically be numbered.

  3. Select File Properties Web formatting. Select the option Create a multi-page form.

    When publishing the web form, select the option Create a multiple-page form on the Web form setup window.

  4. Select Preview to see the form with page breaks.
