Early Electronic Survey Form History
- Raosoft, Inc. introduced its first software for sale in
November 1991, at the COMDEX Show in Las Vegas.
At the suggestion of an agency of the U.S. federal government, Raosoft
immediately began developing software for LAN and network email form distribution. In 1993,
the Environmental Protection Agency sent out the first extensive online survey questionnaire
ever performed in the U.S.

In 1994 the U.S. Agency for International Development distributed
the first online survey performed internationally. Since that time,
innumerable survey projects have been conducted by various agencies.

The first very large worldwide survey was conducted by the Air
Combat Command, U.S. Air Force in 1994.

This was followed by the first electronic Air Force world-wide
survey in 1995, sponsored by the Chief of Staff of the Air Force.

EZSurvey, published in 1997, began the Internet survey era. That
same year, the Department of Veterans Affairs became one of the first
power users of Web-based or Email surveys on the Internet. The Department
of Transportation and the Internal Revenue Service followed in taking
advantage of these extremely efficient new data collection capacities.

Probably the largest online web-based survey in the world is
conducted every two years by the U.S. Air Force through its biennial
worldwide Organizational Climate questionnaire. This project was
initiated by the Chief of Staff of the Air Force for the purpose of
measuring and improving personnel morale. The latest survey was
distributed by the web in September, 2003.
